Patient Portal

As we continue in our efforts to provide you with the highest quality of care, we are constantly looking for methods of working together with you to ensure that you are not only aware of but also involved in the maintenance and improvement of your health.

To that end, we are proud to announce that our practice now offers you the opportunity to use the power of the web to track all aspects of your health care through our office. The Patient Portal enables our patients to communicate with our practice easily, safely, and securely over the Internet.

Through the Patient Portal, you will be able to:

  • ask questions of doctors, nurses, and staff members
  • request prescription refills and referrals
  • request appointments
  • review, print and share your health summary … all from the comfort of your home, whenever it is convenient for you!

By using the Patient Portal you no longer have to call the office, leave a message, and wait for a response to get the results of your lab work; those results will be available to you on the Portal. You no longer have to call with a question or concern; you can send a message to the office through the Portal and expect a reply from the staff.

In order to get access to the portal. You will need to provide us with an email address. We will then send you an email invitation with a link and a PIN number. Just click on that link, create a new account with your name and birth date, enter the PIN provided and immediately get access to your health information!

Appointments & Registration

You can make an appointment by calling our office during regular business hours (8am-5pm) 318-325-2634 or 800-293-2634. If you are an established patient with our practice and have completed registration in our Patient Portal, you may send a request through the Portal to schedule, cancel or confirm your appointment and our practice will contact you. If you are not currently an established patient or have not received an Portal email invitation please call our office.

If you currently have an appointment scheduled and are registered for the Patient Portal we encourage you to complete your medical, family and social history prior to your appointment. Click send to submit your information directly to our office. By completing this information prior to your appointment your arrival and wait time may be significantly reduced.

If you are unable to complete the form via the Patient Portal, please download, print and complete the “Patient Information Form” (see link below). Please fax, mail or bring the form to our office at least 2-3 days prior to your appointment. If you are unable to send the form prior to your appointment, please arrive at least 45 minutes early so that the information can be entered into your medical record.

Insurance Information/Medical Fees

We strive to provide the highest quality of care as efficiently and as economically as possible. The Gastroenterology Clinic's staff is trained to assist you with any questions regarding costs associated with your care.

For your convenience, our insurance staff will file your insurance. You need to bring all insurance information to your appointment. We will work with your insurance company in paying your claim; however, you are ultimately responsible for payment of your account. We accept Medicare and most commercial insurance plans. Please contact us with any specific questions about your policy.

If needed, we will work together to develop a mutually agreeable financial arrangement.
